Address (or place address in body of letter)

Please include the information in bold in your letter.
It is recommended that you send it by certified mail.
You may send a duplicate of the letter of intent and they will
return it to you with their stamp.

The minimum number of days in a school year is to be no less than 180 days.

1This is a letter of intent - it is not applying for approval as a traditional
private school. You are not asking for state approval, simply giving notice that
your children will be homeschooled in your home as a private school and not under
the state home-study program.
